Screening Criteria of Boult et al for Identifying an Elderly Patient at Risk for Hospital Admission

Boult et al identified a number of criteria that can help identify an elderly patient who is at risk for hospital readmission within a 4 year period. This can help identify elderly patients who may benefit from closer monitoring and more aggressive management. The authors are from the University of Minnesota.
